Are you over the age of 50? If so, you are at risk of developing osteoporosis. Complete this questionnaire* to find out if you are at risk for fracture.

|After the age of 40, I suffered a broken bone or a spinal fracture from a simple fall, bump or everyday activity|
|My mother or father fractured a hip|
|I have used glucocorticoids such as prednisone for more than three months in the last year|
|I have a tendency to fall or lose my balance|
|I’ve lost more than 2 cm (¾ inches) in height in the last two years|
